I wrote customer service on Oct 27. Return email on Oct 29 resolved the issue.  
The bar fell off the wall.  I had cleaned the tiles above the bar w/ Dawn dish detergent & distilled vinegar & rinsed with water. 2 days later the bar dropped to the floor.
Customer service asked for more info and sent replacement parts, under warranty.
Included with the install parts was an information sheet.  GRAB BAR SURFACE PREPARATION.  It was a much better examination of install procedures.
1 item was inserting tube end of glue tube and turn clockwise to feel the tube "lock" into place.  Also,  "If chemicals are used for cleaning, use clear caulk around the top 1/2 of the cover ring after installation to prevent the chemicals from soaking the foam rings and possibly damaging the long term bond. Allow the bottom half to remain open for drainage and to promote drying".  
Now this is GREAT customer service.

I've installed eight of their grab bars in various locations. Whenever I've had an issue they've been amazing with information to solve my problem, and even warrantied a problem I had that was probably my fault.
